Really high expectations from junior - inexperienced - programmers. Even though the job was a 9 to 5 one, you had to put lots and lots of extra hours (3+ per day), even weekends, to complete certain tasks "off camera". That resulted in an continuous daily spiral of stress. The old programmers were always too busy to help, although they didn't even offer to in the first place.
